• Be a licensed agent/agency in good standing with the Department of Insurance in the state where you plan to write business.
• Have an active agency Errors & Omissions (E&O) policy with minimum aggregate limits of $1,000,000.
• Sign and attach a Broker Agreement Form and W-9 form to your application.
• Complete the Agency Profile form.

You will need your Property and Casuality E & O policy (Life and Health E & O not acceptable) information and a scanned copy of your state insurance licenses (agency and producer license).
